We would like to thank you for your interest in this team. It would be appreciated if you could download, print, fill out and send back ALL the forms to us as soon as possible.

Team information: MVC is now sending teams to Lesotho and Swaziland to bring relief to AIDS victims through community development. These 14-day mission trips will be building multi-purposed buildings used for churches, sewing co-ops, skills training centres, schools, community centres, food distribution and more. What a life-changing experience it will be!

Please attach to the application a photocopy of the information page of your passport. ( if you don not have a passport yet, make a note on the application form -- still apply now )

If you are under the age of 18 you must have a parent or guardian sign the release of liability form, also the Parent-Guardian Release form and have a Notary Public do the same. You will also find enclosed the Trip Cancellation form. You may want to consider accepting this offer of trip cancellation and complete this form. The insurance covers you if you must cancel your trip due to illness or death of a family member.
