We would like to thank you for your interest in this team. It would be appreciated if you could download, print, fill out and send back ALL the forms to us as soon as possible.

Team information: This team would be a perfect opportunity for a mother and her daughter to join a team and be a blessing to the Nicaragua children. The team are planning on visiting the Re-Nutrition Centre, and a number of feeding centres to tell the children a Bible story, sing some songs with them, do a craft and play games with them. There are many other things that we can do to be a blessing the these dear children that you don’t have to be a Bible teacher or have any special abilities just willing to help and to pitch in where needed.

Please attach to the application a photocopy of the information page of your passport. ( if you don not have a passport yet, make a note on the application form -- still apply now )

If you are under the age of 18 you must have a parent or guardian sign the release of liability form, also the Parent-Guardian Release form and have a Notary Public do the same. You will also find enclosed the Trip Cancellation form. You may want to consider accepting this offer of trip cancellation and complete this form. The insurance covers you if you must cancel your trip due to illness or death of a family member.
