Please attach to the application a photocopy of
the information page of your passport. ( if you don not have a
passport yet, make a note on the application form -- still apply now )
If you are under the age of 18 you must have a
parent or guardian sign the release of liability form, also the
Parent-Guardian Release form and have a Notary Public do the same.
You will also find enclosed the Trip Cancellation form. You may want
to consider accepting this offer of trip cancellation and complete
this form. The insurance covers you if you must cancel your trip due
to illness or death of a family member.
